SABnzbd is skipping duplicate NZB's that are not duplicates

I have been running SABnzbd via a FreeNAS server for some time now.  Had no problems in the past, but now when I manually download an nzb, sab skips over the nzb and does not grab it.  I have a folder set for sab to check for nzb's, and then download them.  But now whenever I add one, I get a warning that it is skipping over a duplicate nzb, when it is definitely not a duplicate. 

I tried turning off the "skip duplicates" option, but I would really like to have that option turned on.  So obviously something is wrong if sab is thinking that a new nzb showing up in the folder is a duplicate.

Any ideas for why this may be happening? 

Running SABnzbd+ 0.4.6 on FreeNAS version 0.69

Duplicate detection is based of nzb file names. Do all your nzb files have the same file name or something?

No, I've even tried downloading the same nzb a few times, but naming it something different each time.  I also get an error that says "Error removing NZB from...", if that helps.

Maybe an  access rights issue?
SABnzbd cannot delete a file after reading it.
