Since tvnzbrss no longer functions for me correctly, I tried setting up an RSS feed using nzbs.org. I have the correct feed generated (with &dl=1 to download), but every item that I have matched has the *, and subsequently is not downloaded.
What am I missing, and why is the * needed at all?
I used the guide below for NZBs.org

Wasn't the text in Config->RSS a clue?
When a feed is added, it will only pick up new items and not anything already in the RSS feed unless you press "Force Download".
The '*' marked items are from the first batch.
They will not be downloaded without you forcing it.

Agreed that it is a clue, but it would be idiot proof if it also said, "The '*' marked items are from the first batch."

Yeah, we don't want to much text there, but it's a bit too concise now.
